When can I register my child?

Return the completed forms to the church office at 204 E. Spring Street, Cookeville, TN 38501.

Registration Checklist:
• Complete a Registration Card for each child
• Complete a Medical Form for each child
• Complete a Volunteer Form for each family

Parent Volunteers are necessary to make this a successful program. There is no charge for this program. We simply ask that every family unit make a commitment of support and effort so that your child has a positive experience.

Parents are to bring their children to the Choir Club Check-In at the entrance to the crosswalk EACH week and pick them up from their classroom EACH week. For the safety of your child, PLEASE honor this request.
